DATA PROTECTION CLAUSE
Health in Code S.L. complies with the provisions on data protection, and by virtue of Organic Law 3/2018, of December 5, on the Protection of Personal Data and Guarantee of Digital Rights (“LOPDGDD”) and of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April, the personal data collected on the website will be treated in accordance with this legislation and incorporated into a file for which Health in Code S.L. is responsible.
The fields marked with an asterisk (*) are mandatory and indispensable to perform the required service. The purpose of the processing is the management of users for the access and use the software “Computerized Register of Hereditary Hemorrhagic Telangiectasia (RiHHTa)” - https://rihhta.healthincode.com - ,for which the categories of data related to the identification of the person will be collected, including the name, contact email, group and work center.
The processing carried out is used exclusively for the purposes established above and collects only data necessary to this end. The data is provided by the interested party and its use authorized by a consent that is expressly granted by accepting these conditions. This consent can be withdrawn at any time.
Said data will not be transferred to third parties without a corresponding prior consent, or in cases other than those expressly defined in data protection legislation.
We also inform you that your data will be kept for the duration of the relationship established with the entity or until you exercise your rights of withdrawal or erasure. In the case of a mere consultation, your data will be kept for a minimum of 5 years from the last notification.
You can exercise the rights of access, rectification, cancellation or deletion, opposition, restriction to processing, and portability of your data by contacting us at: Ed. Please make sure to attach a copy of your ID, passport or equivalent identification document in either case.
You may also pursue your rights before the Spanish Data Protection Agency.
TERMS OF USE LOPDGDD
By accepting these conditions, you also agree to maintain the registry as a system that maintains anonymous patient data for research use. In no case will any identifying data of the patients be stored in any of the software fields, such as names and surnames, identification documents numbers (DNI, Passports, Social Security Number, etc.)
On the other hand, you agree to have all the permits, guarantees and authorizations (if necessary or required) of your work center for the use of the Registry.
